泰国 NBTC 于 2025 年 2 月 3 日公布新的申请表。新的申请表格要求制造商出具信函，指定申请人为授权经销商/代表。

申请人必须提供设备责任方信息如下：
- 商标或品牌所有者证明文件
- 证明申请人是海外制造商授权经销商/代表的证明文件

被修改的声明部分具体如下：
- 添加产品描述和型号
- 添加申请人地址
- 将"We hereby declare with our sole responsibility that below is our authorized representative for our product Thailand approval" (我们特此声明，以下是我们在泰国批准产品的授权代表，对产品负责) 改为  "We hereby declare with our sole responsibility that below is our authorized dealer/representative and responsible for products in Thailand."(我们特此声明，以下是我们在泰国的授权经销商/代表，对产品负责)。
